I confirm that my participation in activities at 5710 Cleveland is entirely voluntary. I am here of my own free will and have not been coerced.

I confirm that I am in good physical and mental health to participate in the activities offered, and I have disclosed any relevant conditions to those I am playing with.

I understand that activities at the venue carry inherent risks, and I accept full personal responsibility for any consequences that may arise from my participation.

I acknowledge that 5710 Cleveland does not perform health screenings, STD testing, or COVID-19 testing on guests. I assume the risk of interacting with others.

I agree to use the venue's equipment in accordance with safety guidelines and only as I have been instructed or as I am personally trained.

I agree to treat all other participants with respect, to honor their stated limits, and to obtain clear consent before initiating any interaction.

I agree to abide by all rules and guidelines of 5710 Cleveland, including instructions from staff regarding safety, conduct, and use of the space.

I release 5710 Cleveland, LLC, its owners, staff, and other participants from any liability for injury, loss, or damage that may occur as a result of my participation.
