I, the undersigned participant, hereby provide my consent to engage in the activities facilitated by 5710 Cleveland, LLC, located at 5710 Cleveland, Cleveland, Ohio. I understand that these activities involve elements of risk and personal discretion, including but not limited to the use of dungeon equipment such as paddles, benches, and other gear.
 

1. Voluntary Participation: I acknowledge that my participation in these activities is completely voluntary and I am aware of the nature of the activities, which I choose to participate in.
 

2. Confirmation of Health: I confirm that I am in good health and do not suffer from any medical condition that could be exacerbated by my participation in these activities. I understand that it is my responsibility to maintain personal health and safety standards.
 

3. Personal Responsibility for Risks: I acknowledge that while engaging in activities at 5710 Cleveland, LLC, I am responsible for assessing my own risk tolerance and comfort levels. I accept personal responsibility for any physical or emotional injuries that may occur during my participation.
 

4. Understanding of No STD/Covid Testing: I understand that 5710 Cleveland, LLC does not provide testing for sexually transmitted diseases (STDs) or COVID-19 for participants. I am aware of the risks associated with not conducting such testing and agree to take personal responsibility for these risks.
 

5. Consent to Use Equipment: I give my consent to use the equipment and facilities provided by 5710 Cleveland, LLC and agree to use such equipment and facilities in accordance with any instructions given and with due care and caution.
 

6. Respect for Other Participants: I agree to conduct myself with respect towards the boundaries and consent of other participants. I understand that any behavior contravening this respect can result in my immediate removal from the premises and possible legal actions against me.
 

7. Agreement to Follow Rules: I agree to follow any rules and regulations set forth by 5710 Cleveland, LLC for the duration of my participation in the activities.
 

8. Release and Waiver of Liability: By signing this consent form, I confirm my understanding of the risks involved and waive any right to hold 5710 Cleveland, LLC responsible for any outcomes resulting directly or indirectly from my participation in the activities.
